Google Forms - Assessment Creation InstructionsOpen Google Drive in silverdaleba.org account
Click “New” in the top left corner and hover over “More” and click “Google Forms”
Create a title for your assessment.
Upper School - it may be advantageous to create a copy of the assessment for each class period, so you might include the class period in the title.
Create a “Name” question if desired. Select “Required” in the bottom right corner.
To grade automatically, click the gear in the upper right corner to enter settings. Click on “Quizzes” and select “Make this a quiz”
Enter your next question, select “Required,” click on “Answer Key” and choose the correct answer.
Assign how many points that question is worth in the upper right corner, and click "Done".
Repeat Step 5 for the rest of your questions.
***At this point your process will change depending on if you teach in Upper School or Lower School. However, the bold words at the bottom apply to everyone.***
Lower School - Sending your assessment to parents:
Click on the eye icon in the upper right corner to preview the assessment, copy the link, and email it to your parents through FACTS.
Upper School - attaching your assessment to Google Classroom:
Open Google Classroom (Either post an announcement or assign an assessment). Click on the desired class and click on “Share something with your class” to post an announcement. Type a message. Click “Add” and then click “Google Drive” in the drop down menu. Select your assessment.
Applies to everyone:
To review student answers, go back to the original form shared with them and click on “Responses.” If you would like a spreadsheet of their answers, click on the green “Create Spreadsheet” button in the top right corner of the Responses page.
Please Note: If you’d like to avoid having ALL of your students’ responses come to the same Google Form, simply make a copy of your quiz and rename the copy to each class period and then assign that copy to the class period.
